The choices we make, individually, affect and inform our health outcomes, and the whole community benefits when we make wise personal choices.
Groups of people and systems matter, too. We must work together to ensure all have access to the quality education, nutritious food, affordable and insightful healthcare and recreational facilities that promote healthy choices.
Our quality and length of life, and that of our neighbors, depends upon it.
The Health Leadership Network, a partnership of healthcare providers, community organizations and County agencies, is beginning to lay the foundation for a healthier future.
A diversity of data have been gathered, documenting Lake County’s health status. While not all of the news is good, understanding where we are is critical to choosing differently, and moving forward in an informed manner.
The Lake County Wellness Roadmap is a way forward.
